Novel Pilot Project Extends Rehabilitation by up to 12 Months at MEDIAN in Germany

AOK Rhineland-Palatinate/Saarland and MEDIAN Germany, part of MEDIAN Group, are piloting an innovative concept using digital app MyMEDIAN@Home, designed to provide targeted and lasting support to patients during and after rehabilitation, helping to solidify recovery progress and ultimately enhance quality of life.

This novel rehabilitation model is set to extend patients’ rehabilitation treatment by up to 12 months with the help of digital support. Therapy is provided via MyMEDIAN@Home app and helps patients regain independence after an illness and promotes long-term Health.

With MyMEDIAN@Home, orthopaedic and cardiology patients at the MEDIAN Rehabilitation Centre in Bernkastel-Kues (the largest rehabilitation centre in Rhineland-Palatinate and the third largest in Germany) can improve their long-term health outcomes. The digital program extends their treatment after they leave the hospital, offering flexible, personalised care at home, which helps them adjust back to daily life. The service is free of charge for insured patients.

Promoting sustainable recovery
MyMEDIAN@Home was developed in close collaboration with doctors and therapists at the MEDIAN Rehabilitation Centre in Bernkastel-Kues, Germany, where a successful pilot phase was completed. This innovative programme, which is unique within the rehabilitation field, is offered to patients in advance of their treatment at participating clinics and patients are made aware via their insurance cover. During their admission to a rehabilitation clinic, they receive a personal consultation where their questions are answered. Rehabilitation begins with an inpatient stay, followed by a smooth transition to continued care at home. Through the app, patients receive customised training plans, relaxation exercises, and valuable medical advice – all supported by experienced therapists from MEDIAN’s Digital Aftercare Centre in Düsseldorf.

“This flexible form of rehabilitation enables participants to integrate their recovery into everyday life – location-independent, self-directed, and always with professional support. Thanks to digital technologies, motivation remains high, while the personal and qualified support ensures lasting therapeutic success,” explains Dr. Martina Niemeyer, CEO of AOK Rhineland-Palatinate/Saarland – The Health Insurance Fund.
The very first patients are already benefiting from this digital method of care delivery. They report an easy integration of the exercises into their daily routine, greater self-motivation, and a new sense motivation to remain healthy and fit in the long term. The ongoing support gives them a sense of security and confidence once they have left the rehabilitation clinic.
